Card Request Confirmation

Please follow the steps to complete your Family Medical Card application.

Upload your family pictures with white background and clearly visible. These pictures will be displayed on the Medical Card.

Pictures of all dependants are required and especially for the whom the card is to be printed.

Cards will be printed for a single dependant and the other dependants will be able to use the same card.
